Our mission is to empower and support individuals with disabilities through advocacy, education, recreational activities, and community engagement. We appreciate your commitment to helping us create an inclusive and welcoming environment.

Participant Registration Waiver and Agreement

As a participant and by submitting this form, I agree to:

• Support the mission and values of The Moe Norris Foundation.

• Treat participants, families, staff, and volunteers with dignity, respect, and inclusion. Misconduct will not be tolerated.

• Follow all organizational policies, safety procedures, and instructions provided by staff.

• Report any safety concerns, accidents, injuries, or suspected abuse immediately to foundation staff.

• Parents of minor children (17 and under) must remain present during the event.

Assumption of Risk and Release of Liability: I understand that participation in any foundation event may include but is not limited to physical activities, sports, games, recreational activities, walking, running, basketball, football, wheelchair games and/or sports, contests, vendor areas, and other event-related activities. I understand that participation may involve certain risks, including but not limited to physical activity, falls, injuries, contact with other participants, dehydration, illness, property loss, or other unforeseen circumstance. I voluntarily assume all risks associated with my participation and my child’s participation. To the fullest extent permitted by law, I release and hold harmless The Moe Norris Foundation, its Board of Directors, officers, employees, volunteers, sponsors, partners, and agents from any claims, demands, damages, injuries, losses, or liabilities arising from my participation as a participant, except to the extent caused by gross negligence or willful misconduct where such limitation is prohibited by law. I voluntarily agree to participate, or allow my child to participate, in this event. I accept full responsibility for any risks connected to participation.

Medical Authorization: In the event of an emergency, I authorize The Moe Norris Foundation, event staff, volunteers, coaches, or medical personnel to seek and/or obtain emergency medical treatment for my child or myself if I am unable to provide consent. I understand that I am responsible for any medical expenses incurred that may result from emergency treatment. I understand that every effort will be made to contact the emergency contact listed on file.

Media Release: I grant permission for The Moe Norris Foundation to photograph, video record, or otherwise capture my image, name, and likeness during MNF activities and/or events and to use these images for educational, promotional, fundraising, social media, websites, press, marketing, and future community outreach endeavors without compensation.

Code of Conduct: I understand that The Moe Norris Foundation reserves the right to terminate my participation and remove my child and/or myself at any time if my conduct is inconsistent with the organization's policies, mission, safety standards, or code of conduct, including but not limited to unsafe behavior, fighting, harassment, threats, disrespect towards staff or participants, or failure to follow event rules. I understand that all participants, parents, guardians, and guests are expected to act respectfully and safely.
