Volunteer Agreement

As a volunteer, I agree to:

• Support the mission and values of The Moe Norris Foundation.
• Treat participants, families, staff, and fellow volunteers with dignity, respect, and inclusion.
• Maintain the confidentiality of any personal or sensitive information learned through volunteering.
• Follow all organizational policies, safety procedures, and instructions provided by staff.
• Report any safety concerns, accidents, injuries, or suspected abuse immediately to foundation staff.
• Perform only those duties for which I have been trained or authorized.

Assumption of Risk and Release of Liability: I understand that volunteering may involve certain risks, including but not limited to physical activity, transportation, outdoor events, interaction with participants, lifting equipment, or other unforeseen circumstances. I voluntarily assume all risks associated with my participation. To the fullest extent permitted by law, I release and hold harmless The Moe Norris Foundation, its directors, officers, employees, volunteers, sponsors, partners, and agents from any claims, demands, damages, injuries, losses, or liabilities arising from my participation as a volunteer, except to the extent caused by gross negligence or willful misconduct where such limitation is prohibited by law.

Medical Authorization: In the event of an emergency, I authorize The Moe Norris Foundation to obtain emergency medical treatment for me if I am unable to provide consent. I understand that I am responsible for any medical expenses incurred.

Media Release: I grant permission for The Moe Norris Foundation to photograph, video record, or otherwise capture my image, name, and likeness during volunteer activities and to use these images for educational, promotional, fundraising, or marketing purposes without compensation.

Background Check Authorization: Certain volunteer positions may require a criminal background check. I understand that, if required for my volunteer role, I may be asked to complete a separate background screening authorization before participating.

Code of Conduct: I understand that The Moe Norris Foundation reserves the right to terminate my volunteer service at any time if my conduct is inconsistent with the organization's policies, mission, safety standards, or code of conduct.

Acknowledgment

I certify that:

• I have read this Volunteer Registration and Release of Liability Agreement.
• I understand its contents.
• I voluntarily agree to its terms.
• The information I have provided is true and accurate.
